Type 2 Diabetes One-to-One Peer Support

Type 2 Diabetes One-to-One Peer Support

Recently diagnosed with type 2 diabetes or just need some support in managing it? We’re here for you.

If you have type 2 diabetes and live in Bristol, North Somerset, or South Gloucestershire, our peer mentor service can help you take control of your health.

We also have type 2 diabetes and want to meet with you to share your experience of living with the condition. We will provide you with the right information and support to help you learn more about diabetes and live well.

You can meet with us weekly and we will:

  • Listen to the challenges or obstacles you’re experiencing
  • Share our own experiences with you of what helps
  • Provide you the information you need about type 2 diabetes
  • Support you to identify goals and work towards them together
  • We can offer both in–person* and online support.

I’ve found the regular mentoring sessions tremendously helpful. The weekly supportive chat has helped me get back in control and made me feel like I can make the changes I need to do to have a healthier lifestyle. Made a vast difference and don’t think I’d be doing anywhere near as well as I am without it. (Mentee, 2021)

If you would like to access peer support, or are a healthcare professional wanting to refer a patient, please complete our referral form here: Type 2 Diabetes Peer Support Referral

If you would like to speak to someone about the service, please call us on  0117 955 5038, email us at info@peerpartnership.org, or complete the enquiry form at the bottom of this page.

Do you have type 2 diabetes and want to help others? If so we want to hear from you!

As a peer mentor, you will receive full training and ongoing support, develop your knowledge of type 2 diabetes, be reimbursed expenses and meet and support other people living with type 2 diabetes.

You don’t need to be an expert in diabetes and you don’t need to have the perfect lifestyle. If you have been living with type 2 diabetes, have struggled to understand the condition, had setbacks and obstacles that have caused you to struggle on the way, you are ideal for this project!

If you’re interested and would like to find out more about the role, please complete the below form, call us at The Peer Partnership at Brigstowe on 0117 955 5038 or email info@peerpartnership.org.
